Piriformis Syndrome Physiotherapy in Clapham
Deep pain in your buttock, worse when sitting or driving, sometimes spreading down the back of your leg? Piriformis syndrome often responds well to the right exercise. Olivia offers thorough, unhurried care at our Clapham clinic.
Understanding Piriformis Syndrome
As part of our pelvis and sacroiliac joint physiotherapy for women, Olivia treats piriformis syndrome in Clapham, helping you ease buttock and leg pain and sit, drive and exercise comfortably again.
What Is Piriformis Syndrome?
The piriformis is a small muscle deep in your buttock, and the sciatic nerve runs close to it. Piriformis syndrome is when this area irritates or squeezes the nerve, causing buttock pain and sometimes sciatica-like symptoms.
The Royal National Orthopaedic Hospital describes it as part of deep gluteal syndrome, where the sciatic nerve is irritated as it travels through the buttock rather than at the spine.
Piriformis Syndrome Symptoms
- Deep pain in one buttock
- Pain that spreads down the back of your thigh
- Tingling or numbness down the leg
- Pain that’s worse when sitting, especially driving
- Pain getting up from sitting or climbing stairs
Piriformis Syndrome or Sciatica?
Both can cause pain down the back of the leg. Sciatica usually starts from the lower back, for example from a slipped disc, while piriformis syndrome comes from the buttock. Pain from the sacroiliac joint or hip tendons can feel similar too. Olivia will assess you carefully to find the source.
What Causes It?
The Royal National Orthopaedic Hospital explains that it can follow a fall or develop from repetitive movements, postural habits, muscle tightness or movement patterns that reduce the space for the nerve. Long periods of sitting often make it worse.
How Physiotherapy Helps
The Royal National Orthopaedic Hospital recommends education, exercise and changes to activities as the first treatment. Depending on your assessment, physiotherapy may include:
- Advice on sitting, driving and activities to ease pressure on the nerve
- Strengthening for your buttock, hip and core muscles
- Gentle stretches and nerve mobility exercises
- Hands-on treatment to ease muscle tension
- A step-by-step return to running, exercise or sport
Sitting and Driving Comfortably
Sitting for long periods is a common trigger. It can help to:
- Take regular breaks to stand and move
- Avoid sitting on a wallet or phone in your back pocket
- Use a cushion that takes pressure off the painful side
- Adjust your car seat so your hips are slightly higher than your knees
Is Stretching Enough?
The piriformis stretch is one of the most searched-for exercises, but stretching alone often isn’t enough, and some stretches can irritate the nerve further. A combination of strengthening, movement and the right stretches usually works best. Olivia will show you what suits you.
What Happens at Your First Appointment
Your first appointment lasts 60 minutes and takes place in a private room at our Clapham clinic. Olivia will:
- Talk through your symptoms, what makes them worse and your goals
- Assess your back, pelvis and hips, your strength and how you move
- Explain what she has found and agree a treatment plan with you
- Give you exercises and practical advice to start at home

Piriformis Syndrome FAQs
- How long does piriformis syndrome take to get better?
Many people improve over a few weeks to a few months with the right exercises and changes to sitting and activity. Nerve symptoms can take longer to settle.
- What is the best stretch for piriformis syndrome?
There isn’t one best stretch for everyone, and stretching alone is rarely enough. Olivia will combine the right stretches with strengthening and advice tailored to you.
- Can piriformis syndrome cause sciatica?
It can cause sciatica-like symptoms, such as pain, tingling or numbness down the back of the leg, because the sciatic nerve runs close to the piriformis muscle.
- Do I need a scan for piriformis syndrome?
Not usually. It’s diagnosed from your symptoms and an examination. The Royal National Orthopaedic Hospital notes an MRI can sometimes help but isn’t always necessary.
- When should I get urgent help?
Call 999 or go to A&E if you have numbness around your bottom or genitals, symptoms in both legs, or difficulty controlling your bladder or bowels, as these can be signs of a serious spinal problem.
- Do I need a referral for piriformis syndrome physiotherapy?
No, you can book directly with us. Olivia is happy to work alongside your GP or consultant if needed.
What our patients say
What I’ve appreciated most is how comfortable and at ease she makes you feel. She is kind, understanding and non judgemental, which makes talking about things that can sometimes feel quite personal so much easier. She always takes the time to listen and explains everything clearly, and the exercises are tailored to what I feel comfortable with.
I’m really grateful for all her support and would definitely recommend Olivia to anyone looking for postnatal or pelvic floor support.
