Pelvic and Sacral Stress Fracture Rehabilitation in Clapham
Recovering from a pelvic or sacral stress fracture and want to get back to running safely? Olivia offers one-to-one rehabilitation at our Clapham clinic, with a step-by-step plan to rebuild strength and reduce the chance of it happening again.
Understanding Pelvic and Sacral Stress Fractures
As part of our pelvis and SIJ physiotherapy for women, Olivia offers rehabilitation after pelvic and sacral stress fractures in Clapham, helping you return to running, sport and exercise safely.
What Is a Stress Fracture?
According to Gloucestershire Hospitals NHS Trust, stress fractures are small breaks in a bone, usually from overuse such as running or jumping. In the pelvis, they can affect the pubic bones (pubic rami) or the sacrum at the base of the spine.
Symptoms
- Deep pain in the groin, buttock, lower back or pelvis
- Pain that builds with running, hopping or impact and eases with rest
- Pain that becomes more constant if you keep training
- Tenderness over the bone
A stress fracture needs a medical diagnosis, usually with a scan. See your GP or a sports medicine doctor if you suspect one.
Why Women Are at Higher Risk
Stress fractures can be linked to bone health. Risk factors include a sudden increase in training, low bone density, including after menopause, and Relative Energy Deficiency in Sport (RED-S).
Oxford University Hospitals NHS Trust explains that RED-S happens when you don’t eat enough to fuel your training, and signs can include irregular or missed periods and stress fractures.
Healing and Recovery
Bone healing takes time, and pelvic stress fractures often need a period of reduced activity, sometimes with crutches, before rehabilitation progresses. Your doctor will guide the early stages, and Olivia will build on this.
How Physiotherapy Helps
Once your doctor is happy, physiotherapy can help you:
- Rebuild strength in your hips, pelvis, core and legs
- Gradually reintroduce impact, from walking to hopping and running
- Follow a structured run-walk return to running
- Adjust your training load to reduce the chance of it happening again
- Get support for bone health, including advice on when to see a GP or dietitian
RED-S and Bone Health
If you have irregular or missed periods, frequent injuries or low energy, it’s important to look at fuelling and bone health. The NHS advises seeing your GP and a sports dietitian. Read more about bone health and menopause.
Returning to Running
Returning to running should be gradual and pain-free. Olivia will use strength and impact tests before you start running, then build a progressive plan. See also our sports physiotherapy.
What Happens at Your First Appointment
Your first appointment lasts 60 minutes and takes place in a private room at our Clapham clinic. Olivia will:
- Talk through your diagnosis, any scans, your training history and your goals
- Assess your back, pelvis and hips, your strength and how you move
- Explain what she has found and agree a treatment plan with you
- Give you exercises and practical advice to start at home

Stress Fracture FAQs
- How long does a pelvic stress fracture take to heal?
Healing times vary. Many stress fractures take around six weeks or more for the bone to heal, and pelvic and sacral stress fractures can take longer before you’re back to full running.
- Can I run with a stress fracture?
No. Running on a stress fracture can make it worse. Your doctor will advise on rest, and Olivia will help you return gradually once it’s safe.
- What is RED-S?
Relative Energy Deficiency in Sport (RED-S) happens when you don’t eat enough to fuel your training. It can affect your periods, bones and performance, and is a risk factor for stress fractures.
- Why do I keep getting stress fractures?
Repeated stress fractures can be linked to training load, low bone density or under-fuelling. It’s worth speaking to your GP, and Olivia can help you plan training to reduce the risk.
- Do I need a referral?
No, you can book directly with us. A stress fracture should be diagnosed by a doctor first, and Olivia is happy to work alongside your GP or sports medicine team.
What our patients say
What I’ve appreciated most is how comfortable and at ease she makes you feel. She is kind, understanding and non judgemental, which makes talking about things that can sometimes feel quite personal so much easier. She always takes the time to listen and explains everything clearly, and the exercises are tailored to what I feel comfortable with.
I’m really grateful for all her support and would definitely recommend Olivia to anyone looking for postnatal or pelvic floor support.
